We utilize a secure client portal for the intake process. When you call to schedule your first appointment, you will be directed to complete these forms in our patient portal.  

If you do not have access to establish an online patient portal, you may print these forms. Please fill them out and bring them to your next appointment. Plan to arrive 15 minutes early.
